We are actively hiring Clinical STARs Pharmacists located in Texas for an upcoming remote project focused on improving Medicare quality outcomes and closing care gaps. As a STARs Pharmacist, you will conduct telephonic medication reviews and outreach to Medicare members to improve medication adherence and close quality care gaps. You will provide comprehensive medication therapy management, screen for drug interactions, educate patients on chronic disease management, and collaborate with providers as needed.
This role operates in a high-volume, productivity-focused environment, with pharmacists expected to complete approximately 17-20 medication reviews per day while maintaining strong documentation accuracy and quality standards.
